That's... a new response, really. Most people, when they are confronted with a rampaging Satan, do not actually attempt any sort of diplomatic negotiation. Then again, normally they're dead by this point, so it's possible that they just never had enough time to try. Either way, the point here is that the Doctor's behaving in a very curious manner, and that's really the only reason why Satan doesn't just hit him again.

"Do I look like I need help?"

The people running away from him in abject screaming horror? They need help. The guy with the fist-sized hole in his chest? Well, he needed help probably about three seconds after Satan removed his hand from his body cavity, but it's really a moot point by now. Long story short, just about everyone else in the vicinity needs help more than the Devil does.
